If you would like to create your own personal electronic health record, please click on the link below. Doing this provides you with important pieces of your medical history that you have entered yourself, such as medications, hospitalizations, surgeries and medical problems.
Currently this is the only electronic function available with us through this portal. In the future we may be able to offer other electronic functions via this portal for you to communicate with the office.
